The following guide will provide instructions for adding an activity to your Moodle site for students to complete their end of semester course feedback surveys. This guide will cover three feedback modules, the activity students will use to complete their surveys, the activity instructors can use to see their class response rates, and the activity that is used to see a report of the feedback provided for their course.



Course Feedback


In order for your students to complete their course feedback surveys, you will need to add the feedback module to your course. This section will cover the creation of the Feedback module your students will use.


1. To start, click the Turn Editing On button



2. Then click on + Add an Activity or Resource



3. From the list of activities available on Moodle, select External Tool



Activity  Settings


4. Fill in the Activity Name as "Course Feedback" or something similar. This is the name your students will see on the Moodle activity


5. For the "Preconfigured Tool" option, open the drop-down menu and select "My Course Feedback" from the list of options



6. Scroll to the bottom of the page and click "Save and return to course"


This will create the Course Feedback activity in your Moodle course. your students will access this activity in order to complete their feedback surveys.



Feedback Reports


This section will cover how to add the Course Feedback Report activity to your course. This module is used to view a report of the feedback provided for you. It will not be used by students and can be hidden from their view. 


1. Repeat steps 1 to 3 listed for the Course Feedback section.


2. Name the activity as "Course Feedback Reports" or something recognizable as this module will only be used by the instructor. 


3. For the "Preconfigured Tool" option, open the drop-down menu and select "Your Course Feedback Reports" from the list of options 



4. Scroll to the bottom of the page and click "Save and return to course"


Your Course Feedback Report module will now be saved onto your Moodle course. You can access this module to view a report of the surveys completed for your courses. 



Feedback Response Rates


This section will cover how to add the Feedback Response Rate module to your course. It is used to view the response rate for the feedback surveys for your courses and can be hidden from student view as it will only be used by the instructor.


1. Repeat steps 1 to 3 listed for the Course Feedback section.


2. Name the activity as "Course Feedback Response Rates" or something recognizable as this module will only be used by the instructor as well, similar to the Feedback Report module. 


3. For the "Preconfigured Tool" option, open the drop-down menu and select "Your Course Feedback Response Rates" from the list of options


4. Scroll to the bottom of the page and click "Save and return to course"


The module to view your Feedback Response Rates will now be added to your course page. Similar to the Feedback Reports module, this one will also only be used by the instructor and can be hidden from student view.
