When creating a Quiz you can either make the questions first and add them to the Quiz, or add a Quiz activity (as below) and create the questions as you go along.
Below are the steps to add a quiz activity to your course.
1. In your course, turn Edit mode On from the top right corner.
2. At the bottom of the desired section of your course, Click Add an activity or resource.
3. Choose Quiz from the activity chooser.
4. Adjust the Quiz settings.
5. Scroll to the bottom and click Save and display.
6. Click Questions in the red navigation menu at the top of the Quiz page.
7. On the right side of the Questions page, click Add and select one of the options.
